How do I put an "ask" page on my tumblr?

when will i put your page about my tumblr if anyone else is to inquire questions Lots of my supporters have these folks.but I don’t discover how to get just one, haha..i’m sorry, I’m sluggish!!:)
I know I need to go in to customize..and then pages..and then I’m ensnared!!:P Regards =) xxx
